Who is in charge of the payment transactions and should have access to the Loxopay account? In the case of a company, this person must either be the owner of the company or a shareholder, a member of the board of directors or someone with significant responsibility in the management. In the case of associations, this person must be someone with significant responsibility in the association.
Passport/ID from the company owners (over 25% company share) or from the association president.
Please upload a colour, complete and currently valid picture of the passport or identity card (front and back) of the company owners (over 25% company share) or of the club president here.
Proof of address (private address) from the company owners (over 25% company share) or from the association president.
Please upload a proof of address here showing the date, name and home address of the company owners (over 25% company share) or the association president.
The document must not be older than 3 months. Only the following documents are accepted:
- Electricity or water bill
- Confirmation of residence, extract from the debt collection register
- Bank statement or credit card statement
Unfortunately, all other documents cannot be accepted. Please note that the entire document must be visible without parts of it being cut off. Sensitive data may be blacked out.
Additional owners
Regulations require us to record information on all owners of the business. Owners are defined as persons with shares in the business of 25% or more.
Please list all holders with the following information:
-First and last name
-Email address
-Percentage share
-Job title (CEO, CFO, etc.)
-Member of the Board of Directors?
To which bank account should the payments be made?
Please enter the bank information to which the shop income is to be paid out. The account holder must match the company name/association name so that a payment can be made. The income is automatically paid out at the end of each month to the this bank account. A payout includes all payments made up to eight days before the payout date.
